- If the healthcare team is unable to resolve your concerns, ask to speak with a manager where you’re receiving care.

Our Patient Relations team is available to facilitate a formal review and follow-up of your concerns. Information shared with us is reported to the leadership team, so we can ensure we’re providing exceptional care organization-wide.

Patients, family members, and visitors may talk with a Patient Relations representative at 503-681-4357 (HELP). Our representative is available by phone Monday – Friday from 8:00 a.m. to 4:30 p.m.

Patient Privacy Breach Notice
On June 24, 2022, the Information Privacy and Security Office (IPSO) received a report that a Hillsboro Medical Center employee may have inappropriately viewed a patient’s health information. The information included: full name, date of birth, address, phone number, providers’ names, dates of services, medications and diagnoses. The information accessed did not include the patient’s financial information or social security number.
Despite exhausting all possible contact options, IPSO was unable to directly notify this patient due to out-of-date contact information (no known home address, phone number, email address, and MyChart account not activated).
