Information & Forms

From helping patients fill out forms and answering billing questions, to working with insurance companies and providing referrals, at the Tuality/OHSU Cancer Center we do everything we can to ensure our patients have a positive and pleasant care experience. On this page, you’ll find useful information to guide you during your visit.

Billing Questions

At the Tuality/OHSU Cancer Center, we understand billing statements may be confusing and lengthy. That’s why our team is always happy to answer any questions patients may have regarding their bill and payment options.

For all billing questions, please contact our Patient Accounting office at 503-681-4248.

The Tuality/OHSU Cancer Center accepts most major insurance providers. We recommend that patients confirm with their provider before their visit to ensure we’re in-network and determine if any additional referrals are required by their insurance. We ask that patients provide us with copies of their insurance card and billing information so we may bill their insurance provider for them.

If you have questions regarding our accepted insurance, please contact our Patient Accounting office at 503-681-4248.

Financial Aid

We are committed to meeting our patient’s healthcare needs and treating them with compassion and understanding. The Tuality/OHSU Cancer Center offers financial assistance to uninsured or underinsured patients who qualify under Tuality Healthcare’s financial assistance program. Assistance will vary depending on individual financial circumstances.

If you would like to be considered for financial assistance, please contact our billing office or reach out to our patient advocate for information and direction at 503-681-1000.

Patients with an emergency or an urgent medical problem after-hours should call 503-681-4200 to reach the doctor on call.

In case of a life-threatening illness or injury, call 9-1-1 immediately or go to the nearest hospital emergency room.

Lab Results

Laboratory tests provide valuable information for making healthcare decisions. We use them to help assess a patient’s health status, diagnose disease, determine appropriate treatment or monitor progress.

If you are a current patient at the Tuality/OHSU Cancer Center, you can view your lab results by using the online health management tool, Tuality MyChart.

Prescription Refills

To get your prescription refills ready, please contact your pharmacy during weekdays and give them two days notice. Once they fax us the prescription request, we’ll make sure everything is accurate and process it.

For prescription refill questions, please contact our office at 503-681-4200.

If a patient’s physician thinks they need specialty care, they will be referred to the appropriate specialists. Many insurance companies now require a referral from a primary care doctor prior to seeing specialists.

If you need a referral, please contact our office at 503-681-4200 in advance.
