Hillsboro Medical Center is committed to providing you and your loved ones with a respectful and positive experience. Our patient relations team listens and helps address your concerns to ensure you and your family have safe, high-quality and compassionate care.

If your expectations are not being met, we encourage you to take the following steps:

  • Talk to your healthcare team about your concerns.
  • If the healthcare team is unable to resolve your concerns, ask to speak with a manager where you’re receiving care.
  • If your concerns remain unresolved, please contact Patient Relations at 503-681-4357 (HELP) for further assistance.

Patient Privacy Breach Notice

On February 6, 2024, the Office of Information Privacy and Security (OIPS) discovered that a Hillsboro Medical Center workforce member accessed a patient’s medical record without a work-related purpose. The health information included: name, date of birth, address, phone number, medical record number, providers’ names, dates of services, medications, diagnoses and other treatment information. The information accessed did not include the patient’s financial information or social security number.

Despite exhausting all possible contact options, OIPS was unable to directly notify this patient due to out-of-date contact information (no known home address, phone number, email address and MyChart account not activated). The impacted patient last received services from HMC in April 2015.

Please contact the Information Privacy and Security Office at 503-494-0219 or privacy@tuality.org if you have any questions about this post.

Survey

After your visit, you may receive a phone call from the National Research Corporation (NRC Health) inquiring about your experience.  We encourage you to provide your feedback so we can improve the services we provide to you, your family, visitors, and the community.
