
Hillsboro Medical Center, an OHSU Health partner, has achieved Tier 5 recognition as a Patient-Centered Primary Care Home by Oregon Health Authority for all four of our primary care clinics.
Patient-Centered Primary Care Homes are health care practices recognized by the Oregon Health Authority for their commitment to providing high quality, patient-centered and equitable care.
Tier 5 is the highest recognition the Oregon Health Authority quality program awards.
“This recognition reflects Hillsboro Medical Center’s dedication to improving access to care and fostering strong relationships with patients to better treat the whole person,” says Director of Ambulatory Services Dani Proctor.
Hillsboro Medical Center’s primary care clinics – 8th Avenue Primary Care, Forest Grove Primary Care, Orenco Station Primary Care and South Hillsboro Primary Care – provide comprehensive care for approximately 25,000 patients throughout Washington County.
To achieve Tier 5 recognition, Hillsboro Medical Center made dedicated improvements to meet the program’s high standards around timely access, accountability, and comprehensive whole person care.
Some of the implemented measures included:
- Extending clinic hours and offering same day access for patients
- Expanding behavioral health services
- Providing telephone triage with a registered nurse
- Hosting a Patient Family Advisory Council twice a year
- Adding an on-site clinical pharmacist and more community health workers
Earning this Tier 5 designation represents Hillsboro Medical Center’s continued commitment to building a healthier community by bringing quality clinical care and unparalleled service to our region.
