Requesting Medical Records
Any patient may review or obtain a copy of his or her medical records. You may also have copies of medical records sent to another medical provider or facility.
To protect your privacy, Hillsboro Medical Center requires a completed and signed written request and authorization before we will release any medical records.
We ask that you share which parts of your medical records you wish to obtain. Common record requests include: the discharge summary, the operative report, the history and the physical.
Note that appointments to review or obtain copies of your medical records in person are required. Call Medical Records at 503-681-1195 to schedule your in-person appointment.

Where can you request copies of film records or CDs?
To obtain copies of radiology films or CDs, contact our Diagnostic Imaging Department.
