Hillsboro Medical Center, an OHSU Health partner, has attained Level I Age-Friendly Health System status for our hospital through the Institution for Healthcare Improvement.
All four of our primary care clinics have also been recognized as a Level 2 Age-Friendly Health System — Committed to Care Excellence through the Institution for Healthcare Improvement. Level 2 is the highest recognition that the Institution for Healthcare Improvement honors.
These two distinctions note that Hillsboro Medical Center follows the 4Ms framework of evidence-based geriatric best practices.
The 4Ms are:
- What Matters: Know and act on each patient’s health goals and preferences.
- Medication: Use medications that support goals, values and preferences while minimizing the impact on mobility and mentation.
- Mentation or mental acuity: Prevent, identify, treat and manage dementia, depression and delirium.
- Mobility: Ensure patients move safely every day to maintain their function.
A few examples of how the 4Ms are delivered at Hillsboro Medical Center include:
- Assessment of the daily and overarching health related goals of older adults and working towards achieving them.
- Responsible medication management to identify potentially inappropriate medications (PIMs) for older adults and adjust or discontinue them as needed
- Routine delirium screenings to prevent and identify changes in mentation
- Completion of mobility assessments to ensure patient safety and encourage mobility
