HILLSBORO, Oregon — Hillsboro Internal Medicine, a member provider of Tuality Healthcare, an OHSU Partner, received Tier 4 recognition from the State of Oregon’s Patient-Centered Primary Care Home program on Tuesday, October 24.

“This recognition is a testament to the hard work of our staff and providers in delivering outstanding primary care to our patients,” said Joe Hardman, M.D., Chief Medical Officer of Tuality Healthcare.  “In particular, I would like to thank senior practice manager Nicole Watson and medical home specialist Tonya Magwood for their tremendous work on this effort.”

A Patient-Centered Medical Home is a team-based health care delivery model intended to provide comprehensive and continuous medical care to patients with the goal of obtaining maximized health outcomes.  The Patient-Centered Primary Care Home program is part of the State of Oregon’s effort to fulfill a vision for better health, better care, and lower costs for all Oregonians.

Hillsboro Internal Medicine is a patient-centered primary care home in downtown Hillsboro.  Tuality Healthcare primary care clinic physicians focus on the wellness of the patient and coordinate care with a broad team of health professionals.
